1/11/2023 EnchantaVaughn Approved Upon arrival, client was supported for being in his seat, completing his work, and avoiding distractions in class. Client was focused and on task for the duration of the morning hours. Client had a small debrief with counselor during his first check in. Client was inqured about his morning and how he was feeling. Client said he was working hard and feeling good. Client said he was looking forward to todayâs plans. Client confirmed he didnât need anything from QMHP but to take a break when he finishes all of his work. Counselor agreed and prompted client to utilize coping strategies to deal with difficult tasks. Client agreed to try his best to avoid aggressive and impolite behaviors and comply to rules at all times. Client was observed during lunch time of showing respect and kindness. Client had appropriate interaction with peers. Client was having appropriate conversation too. Client was respectful of staff and peers. Client was helpful to his peers, as evidenced by him picking up trash that wasnât his. Client was courteous to authority requests and demands. Individual therapy was scheduled to assist client in adjusting to his improved performance. Client shows great attitude and high level of participation. The client has become less defensive and has not shown any recent incidents of unreasonable anger. Client understands his expectations and leads by example. Client is observed displaying age appropriate manners, avoiding distracting behaviors. Client continues to build new relationships in the program. The client identified a variety of successes and difficulties in social settings and was provided with support and feedback in this area. The client was provided with positive feedback as he displayed a realistic understanding of his recurrent pattern of mental illness symptoms.
